Contact For Plugin question

I'm looking for a good contact form plugin for wordpress that allows you to have different fields the surfer will have to fill.
EX:
The usual
Name:
Email:
Subject:
and instead of a main message box, I would like to have different fields where the surfer would enter specific information
EX:
address:
city:
country:
phone number:
etc...
I tried conact form 7 plugin and I'm not getting the results I expected.
