David did it himself. He used an on-site monitor which actually shows the 3D effect ( a Fugifilm 3D viewer, shows 3D without needing any glasses)
You can adjust the parallax after focus is locked, by previewing it in the monitor and then adjusting in or out, plus we used StereoPhoto Maker (great software) to fine adjust the parallax and create the various output formats. Its really not that hard. And the video is shot in what is called Stereo AVI, which is a regular AVI with 2 video streams, same basic concept as the photo stuff.
