Set a cookie which controls which join forms are shown. If neither the special Epoch or CCBill affiliate cookie (special for your site) is not found then show it as normal as you would for a type-in. Make sure to set the cookie length for these cookies equal or greater than the length of the respective affiliate cookies.
I think you might also want to intercept the redirect from refer.ccbill.com (and epoch's) and then set the appropriate cookie at that point, possibly based on the referrer.
Just be careful not to overwrite any of the regular CCBill or Epoch cookies anywhere.
CCBill also is supposed to have a cascade with Epoch that when properly set up gives affiliates credit. I'm not sure if Epoch has the same thing. for their affiliate program.
