1. Upgrade to newer plan involves setting a new server and transfer all content.
2. We will made the transfer from other provider (that is included in the setup fee). We will need ssh acces your old server/account. Regarding tunning and speed all of those are included in setup fee. All things regarding server will be made custom for your needs but we don't do php/css/html coding.
