Usually the cause is prostate and is often due to a tear in one of the veins of the prostate (prostatic varices). Also this is most likely in your case.
Detailed medical history, physical examination, PSA levels (prostatic specific antigen), prostatic ultrasound, per rectal prostate examination, examination of the prostatic fluid, laboratory tests, transrectal ultrasound examination of the prostate, MRI, CT, and cystoscopy are usually done to reach a diagnosis. Evaluation for hypertension, chronic liver disease, amyloidosis, lymphoma and bleeding disorders will need to be done as well. These systemic diseases increase the risk factors for hematospermia.
