And that is - unique IP capping . While almost every ad network have "user" capping, problem with that method is how is based on cookie , so if user deletes his cookie, he will see an ad again, and advertiser being charged for "unique user" which is not unique. So, an shady publisher could simply set a script using his own ip to keep refreshing own site, and deleting cookie every time on refresh , and he will be seen as unique user always. While some ISP'S have dynamic ip's and it is possible to be different user with same ip in 24 hour period, in most cases is a pure bot traffic. So far it seems only one ad network have this feature.
