I read that last night. It initially sounded like he inhaled cooking cod for hours to have that reaction. He was severely allergic to cod.
The fact he had a known fish allergy should have made the parents not have fish in the house. Plus an Epi Pen available in case of allergic reaction is common sense. All other children should have been trained in treatment of allergic reactions. A doctor should have told the parents to keep him away from fish and fish smells. If they did, then reinforce that fact
