I've heard stories like this happening in a few different armies so I'm willing to bet this is more common than most people think. Still scary though!

Hardly a fail, more a success, the grenade throw failed but the trenches and the instructor saved lives. Impressive.

That happens more often than you think. It happened in another unit while I was in Basic Training for the US army. It's the reason why they dig that trench around the position the way that they do. New recruits just get really nervous with the thought that they have a live explosive in their hands with only 5 seconds before it explodes, and they screw up.
