pre 9/11 from what i remember there used to be a huge community (when i was in college...and needed that type of shit) for templates, instructions, etc....almost all of it is gone...from the web anyways.

I have seen some people selling them on Craigs List, check there. They are also selling fake insurance cards. With technology today, I think its way harder to get a decent one that will pass for cheap.
